DISABILITY REVIEW SERVICES
★ Provide proactive medical case management from initial TANF eligibility through SSI/SSD application and appeal
★ Timely gathering of medical documentation and appointments to support favorable social security determinations
★ Evaluations of medical evidence to ensure appropriate return-to-work parameters, if applicable

EMPLOYMENT SERVICES
★ Collaborate, implement, and monitor Individualized Employment Plans (IEP) by establishing realistic goals and objectives
★ Coordinate referrals to appropriate employment support services
★ Maintain accurate and timely progress notes in Case Management System (CMS)
★ Facilitate career development and workplace skills
★ Monitor client attendance in Work Experience Program (WEP) to ensure compliance
ACCES-VR
★ Coordinate the intake, assessment, and enrollment of participants
★ Develop individual placement plans that are consistent with IPE goals and abilities
★ Provide training in resume development, cover letters, and interview preparation
★ Offer tax incentives to employers, to include WTO and OJT
★ Provide job development, placement, and 90-day retention follow-up services
★ Maintain active employer database matching positions with the skills of participants
